An update regarding the work (at Karolinska University Hospital) with the above mentioned patient reported form for chemotherapy-related symptoms.

The design allows for reuse of questions and associated response alternatives (as embedded reusable templates) in several different questionnaires. (An important requirement for scalable reusability in practice here.) If this works out well I think the pattern could be included in discussions about future versions of the "Symptom/sign screening questionnaire” archetype and associated archetypes.
We also supply a reusable follow-up-question archetype.
