Statement of exclusion of all vaccines - is it a global or specific exclusion?

I’m trying to make a statement that a patient has never had any vaccines. There are 2 ways I can see this taking place:

  1. Using the global exclusion archetype
  2. Using the specific exclusion archetype

Could anyone shed some light on which is the correct archetype to use and how it should be used to make the intended statement?

The way I understand those two exclusions, is that for your case I would have used the global exclusion for the statement of never taken any vaccine (regardless of what the disease the vaccine is for) and the specific exclusion to rule out the individual has never taken any vaccines against, say Covid-19. As per now you’ll have to use the ‘Medication’ category, while it can be discussed to add a ‘Global exclusion of vaccination’ RTNC or Coded text alternative in the future (see below).

However, there are change requests to both archetypes, which will most likely result in a major redesign. One is to change from EVALUATION to OBSERVATION, among others are changing from Runtime Name Constraint (RNTC) to Coded text for the elements where there are RTNC.

Yup. I agree . This is a global statement.

Tbh i think the need for specific exclusions has largely disappeared and that questionnaires are a much better approach.

The only exception is Ithink the need for a diagnosis exclusion to handle the rule out diagnosis like ‘meningitis excluded’.
