Request Regarding Timing Element in OBSERVATION "Social context screening questionnaire" Archetype

Dear CKM team,

We are currently in the process of modelling a template to collect discharge reports from various health centres in our region. In accordance with our Ministry of Health’s guidelines, we are required to persist, among other elements, data related to the timing of social situations relevant to the patient.

We have considered using the OBSERVATION archetype “Social context screening questionnaire”, but have noticed that it no longer includes the “Onset of any social issues” data element. We are considering submitting a change request to include a timing element — specifically, the ability to record both start and end dates — but have seen that this was intentionally removed a few years ago.

Before proceeding, we would appreciate understanding the rationale behind its removal. Is there a specific reason we should be aware of?

Do you have any examples of social situations?

I’m also wondering what “social situations” could be.

Generally, the screening questionnaires are not supposed to be the structure to store persisting information. It is said in their Use section (in this case “Exposure”, but is’s similar in all of them):

"This archetype has been designed to be used as a screening tool or to record simple questionnaire-format data for use in situations such as a disease registry. If the screening questionnaire identifies the presence of exposure it is recommended that clinical system record and persist the specific details about the exposure using a relevant exposure archetype, for example the EVALUATION.exposure to record details about the actual exposure."

So, the social situations should rather be stored for persistence in archetypes designated for that/those specific social situations.

We have opted to use a screening questionnaire, as we are currently compiling a list of antecedents such as social support, homelessness, or responsibility for a dependent relative. For this reason, the use of an EVALUATION archetype does not align well with our current use case.
We do not aim to persist the detailed information, but only the screening data, along with a start and end time, if applicable.

I’m sorry, but I do not understand this. A mindmap or a template would help to understand your case better, along with an example or two of the data you plan to store.

The element was removed in revision 2 (July 2020), so very early on in the development of screening questionnaire archetypes. Since then the pattern of this family of archetypes has evolved to include a more generic “Timing” element which I think could well be included in this one as well. See for example Problem/diagnosis screening questionnaire.

Maybe you could add a change request about this, @ClaraCalleja ?
