Actions can happen on their own as well - **Unplanned Actions.** (Acts of God, Earth quakes, (road) accidents, lightning, etc, etc) **Documentation patterns** In EHR's healthcare providers document the process of the provision of healthcare for a particular patient system. The **patient system** is the patient as a whole and can include its context (physical or social). In the patient system processes change the state of thing in the patient system all the time. There are (at least) four distinct types of archetypes, types of Documentation Patterns that can be used to document the provision of healthcare. - To document something about the **state** of something in the **patient system**, at point(s) in **time in the past**, using a specified **method,** we have to use the **Observation Documenting Patter**n. - To document something about a **process** in the **patient system** at **this point in time**, we have to use the **Evaluation Documenting Pattern**. - To document something that is **intended** to **interact** with the **patient system,** at a **future** **time,** by **issuing orders** for **Actions**, we have to use the **Instruction Documenting Pattern**. - To document something that has **changed** the **state** of the **patient system,** at a **point(s) in time in the past,** we have to use the **Action Documenting Pattern.** The key elements are in **bold**.
