# Template Designing **Category:** [Clinical (archive)](https://discourse.openehr.org/c/clinical-archive/153) **Created:** 2008-07-08 14:30 UTC **Views:** 4 **Replies:** 2 **URL:** https://discourse.openehr.org/t/template-designing/12981 --- ## Post #1 by @Paria_Kashfi Hi all, well, after spending some days on Archetypes and their relations to Templates, now, I want to start creating a sample Template using the Template Designer. We discussed about the methodology before , and since I have a previously designed form -however not based on openEHR- in hand, I'll start mapping this form to a Template. So, I don't need Brain Storming or even organizing data, I guess I just should find appropriate Archetypes, and of course the main one for this template my existing form, gathers information related to Salivary Gland, and is called SALIVARY GLAND EXAMINATION including these questions: 1- What areas were examined? (Labiale, Parotis, Sublingualis, Submentalis ) 2- What is the color of skin/mucosa covering the salivary gland? (Bluish, Erythematous, Normal, Pale, Red) 3- Are there any sign of mucositis? 4- Does the patient have a lobulated tongue? 5- are the swelling of gland uni- or bilateral? (Bilateral, Unilateral) 6- Can saliva flow form ducts be seen clinically? 7- Does the patient have a lot of cavities? 8- Does the patient have a swelling of the lacrimal gland(s)? 9- Has roentgenological examination revealed signs of sialoadentists? (No, Scientigraphy, Sialography) 10- Unstimulated saliva 15 min (mls/min) 11- Note As one who has very few knowledge in medical domain, I just search for keywords like skin, cavity, eee to find suitable archrtypes! Can any one help me finding proper archetypes for this template? I am working away on a preliminary Mindmap for a 'Salivary Gland Exam' template to show where new archetypes are probably required and where existing ones can be reused\. I will try and make this available within the next few days\. I want to take my time as it dose highlight some interesting design issues and principles\. Ian --- ## Post #3 by @ian.mcnicoll As promised This is far from perfect but should give you some idea of how to transform a questionnaire type form to reuseable archetypes. For discussion!!
