As a start point, we have tried to tease out the various 'evaluative' elements of a patient encounter and in problem\-orientated health issue/concern threads and have posted an entry to the openEHR wiki at http://www.openehr.org/wiki/display/healthmod/Problem%2C+Issue%2C+Diagnosis+and+Concern We would be grateful for some feedback on these concepts, prior to incorporating them into related CKM archetypes\. The focus for now is on identifying the separate concepts, rather than getting embroiled in naming\. Please use the CKM discussion area from the 'Problem' archetype at http://www.openehr.org/knowledge/OKM.html#showArchetype_1013.1.169 to make any comments as this will lead to a more interactive discussion\. I will monitor for any comments made directly on the wiki\. You will need to register \(free\) with CKM at www\.openehr\.org/knowledge, if you have not done so already, then adopt the 'Problem' archetype\. This will ensure that you are notified of any new comments\. Problem and Diagnosis are clearly two of the most significant archetypes that should be standardised across the international community and we need your assistance to make this a lively and productive debate\. They are refered to in the text, but not included in the reference list. Normally an approach in the academic world of publishing and referring. I will add the Contsys reference - what is the best reference for the HL7 Concern tracking material. I found it quite difficult to establish a definitive location where the various relevant discussions and evolving standard were documented?
